A primary care physician is a first-contact, responsible for providing and coordinating your medical care on all levels. In managed care, primary care physicians are sometimes called “gatekeepers,” because they treat most medical problems and direct patients to specialists when needed.

Dvinder Kaur, MD graduated with a Bachelor of Arts (BA) in Biology from Franklin & Marshall College in Lancaster, PA. She received her medical degree (MD) from University of Medicine and Dentistry of New Jersey — New Jersey Medical School in Newark, New Jersey. She completed her Internal Medicine residency at UMDNJ – Robert Wood Johnson University Hospital.
After residency Dr. Kaur was on faculty as an assistant professor in the Department of General Internal Medicine at the UMDNJ (now Rowan University) School of Osteopathic Medicine in Stratford, New Jersey.
She has been practicing in Middlesex County since 1998.
Dr. Kaur is Board Certified in Internal Medicine.

Appointments can be made by calling (732) 545-8500 from 8:30 am to 4:30 pm, Monday to Friday.
If you have a medical problem that needs immediate attention, we can schedule you for a same day visit. Walk-ins can be accommodated based on the availability of our schedule.
Please bring all medicins, including prescriptions, over the counter medication and vitamins.
If you are coming in for a physical exam or bloodwork for cholesterol or diabetes checks, please remember to fast (No eating or drinking after midnight)
If you are unable to keep an appointment, please notify the receptionist as soon as possible.
We will try to see you at the appointment time. We realize that your time is valuable and will do our best to keep you from having to wait for a long time.
Please arrive TWENTY MINUTES prior to your appointment to give our staff the time to place your information in our electronic medical records system.

IMPORTANT: If you have an HMO please ensure that Prime Medical Associates, Dvinder Kaur MD is listed as your PCP (primary care physician) on your insurance card.
Your appointment will be canceled and you will not be seen if we cannot verify that our practice is your PCP at the time of your visit.
If you have not already arranged for your medical records to be transferred, please do so as soon as possible. Forms will be available in the office to request your records if they are not in the office at the time of the visit.
We ask that you present your insurance card to us at every visit. If you fail to provide us with the correct insurance information at each visit, you may be responsible for payment for all services provided.
We accept most insurance plans including several HMOs, PPOs, POS, Medicare and Medicaid. We also accept many other commercial insurance that are not affiliated with an HMO or PPO. Please call our office if you do not see your plan listed.

Most major health insurance carriers require participants to obtain a referral authorization from his/her primary care physician (PCP) before seeking care in a specialist’s office. You may be required to see your physician before your insurance will authorize it.
It is the patient’s responsibility to notify Prime Medical Associates of a referral request prior to seeing a specialist for medical care. If your plan is an HMO product, please call the office to request a referral. If you are unsure whether your plan requires referrals, please check with your insurance company directly.
Most referrals can and will be processed within 72 hours. Same day referral requests will only be processed if authorized by the physician as an emergency.
No referrals can be backdated.
PROOF OF INSURANCE
We ask that you present your insurance card to us at every visit. If you fail to provide us with the correct insurance information at each visit, you may be responsible for payment for all services provided.
FEES AND PAYMENT
We request payment at the time of your visit. Any copays or deductibles are to be paid at time of service. If payment in full is not possible, please contact our billing office and they will make payment arrangements with you.
If you are a Medicare patient, your charges will be billed directly to Medicare. We will accept assignment of benefits. You are required to have a valid Medicare identification card in your possession at the time of your visit. Remember that you are responsible for your deductible and will be expected to pay for charges not covered by Medicare.
SELF-PAY PATIENTS
You are responsible for full payment of charges at the time of service. All charges must be paid.
HEALTHCARE SAVINGS ACCOUNT AND HIGH DEDUCTIBLE HEALTH PLAN PATIENTS
Patients enrolled in these types of plans are responsible for the payment of the deductible for treatment and services received.
HOSPITAL CHARGES
We accept assignment of benefits for most in-patient charges and we will bill your insurance company. You will be notified of any balance due after your insurance pays.
METHODS OF PAYMENT
We accept cash, checks, VISA and MasterCard. We do not accept post-dated checks, nor will we hold checks for any length of time. Prime Medical Associates will charge a $35.00 billing fee for all returned checks.
An Internal Medicine physician or “Internist” is a specialist physician concerned with all aspects of adult medicine, excluding surgery and advanced gynecology. Their training is dedicated to learning about the prevention, diagnosis and treatment of diseases primarily affecting adults. Their concerns include such diverse problems as arthritis, hypertension, heart and disease, diabetes and thyroid disorders, stress and depression, obesity, as well as nearly any other illness, acute or chronic, minor or major that may befall an adult.
They see patients for all types of symptoms, physical or emotional complaints and illnesses throughout all phases of their lives—from adolescence to geriatric, from office to hospital to intensive care to the nursing home. When other medical specialists are involved it is the Internist who takes responsibility to coordinate the care of the patient and manage the complex medical and social problems associated with their care.
Through analysis and interpretation of a patient’s medical history, physical condition and the results of diagnostic testing, the internist is trained to reach a conclusion by considering even seemingly unrelated clues and to then formulate a treatment protocol.
They are also responsible for the ongoing supervision of healthy individuals, counseling patients of appropriate lifestyle and behavior modification to prevent and manage diseases to provide the highest quality of health.
Our goal at Prime Medical Associates is to work together to deliver the entire spectrum of Internal Medicine to patients – prevention, care for acute illness, chronic disease care—and in order to support patients as they make informed decisions in an era of increasingly complex testing and treatment options and to coordinate their health care in the office, during hospitalization, intensive care, in-care settings and at the end of life.
